What to expect at your Certification Appointment
At your certification appointment, Dr. Filiberto will talk with you about your qualification for certification of a medical condition that may qualify you for the Florida Medical Marijuana Program.
In order to receive the email from the Florida Medical Marijuana Registry, patients must be a Florida resident or a seasonal resident. Florida Residents must present a valid Florida ID, such as a State issued Florida ID or a Florida driver’s license, along with the physician visit fee. Florida Seasonal resident must present with a Florida State ID or a current utility bill. Please note that insurance companies do not cover medical marijuana consultations or the state’s medical marijuana application fee.
By law, Dr. Filiberto must only discuss your qualification for the program. After review and approval, qualified patients will receive an email from the Florida office of Medical Marijuana Use that will allow them to apply to the state for a Medical Marijuana Identification Card.

APPLY FOR YOUR ID CARD
- Retrieve your Email from the Office of Compassionate Use Registry. (check spam folder) Follow instructions on how to apply for your ID card.
- The patient and legal representative will need an ID card prior to purchasing their order. Once you receive your patient registry number that starts with a “P”, you will need to complete the Registry Card Application online through your initial email from the MMU. To do this, you will receive an Email from the Office of Medical Marijuana Use Registry. Open this email and follow their instructions.
- Patients will need to upload a passport quality photo, proof of FL residency (Florida Driver’s License in full color), and mail a check or money order (certified receipt to track it) payable to the Department of Health for $75. If the application is done online or by paper form correctly, the patient will receive a temporary ID Card via email 21-30 days from the date that they receive your check or money order. At that time you will be able to purchase your medication while waiting on the card to be mailed. Patients must have their Compassionate Use ID card or temporary ID card email to purchase their recommendation. Compassionate Use ID cards must be renewed yearly.
- If you have not received an email from the Medical Marijuana Registry within a two days call our office to verify your email is correct the you may go to mmuregisrty.flhealth.gov and enter your email and reset your password.
- Once the patient has received their ID card they may fill their order with any dispensing organization or walking into a local dispensary near them. Patients must have their Patient ID Card. Some dispensaries deliver statewide.

The first step in the medical marijuana application process is documentation of your medical condition. Without current and verifiable medical records, we will be unable to certify your condition under Florida Medical Marijuana law. Although there are conditions where a patient will qualify for the Florida Medical Marijuana Program without the necessity of full documentation, Dr. Frank Filiberto requires that all your records are physically signed or electronically signed by a MD. documenting your qualifying condition.
If you arrive to your appointment without these records, your appointment will be rescheduled. This will delay the process of obtaining the medical marijuana recommendation needed to use cannabis legally.
Step 1:
Contact Your Primary Care Doctor or specialist and request office note/progress notes and any MRI or CT scan reports pertaining to your qualifying condition. The records must be signed by a medical doctor. Unfortunately an ARNP signature, chiropractic notes, and or summary from patient portals can not be accepted per Florida State Law.
Most practices require you to fill out a specific form to formally request your medical records. To obtain this form, simply call your primary care doctor’s office, or the office of the doctor who gave you the diagnosis, and request a copy of your records. They should be able to deliver it to you by fax, email, snail mail or you may pick it up from the doctor’s office.
If the doctor’s office doesn’t have a specific form, simply write a letter to request your records. Be sure to include:
· Your name, including your maiden name (if applicable)
· Social security number
· Date of birth
· Address and phone number
· Email address
· Record(s) being requested
· Dates you were under the doctor’s care
· Signature
· Delivery option (pick up, fax, email, etc.)
If you do not have a primary care doctor, are uninsured, or, have not had a check-up in a while, Please see a primary care physician to evaluate and document your medical condition to determine if you have a qualifying condition.
